1) Complete Application Form

2) Send $400 deposit to save your son’s spot

3) Download and Fill out prior to start of the trip:

1. Parent Consent form
   • Must be notarized
2. Rules of the Summer Trip
   • Signed by you and your son

COST

We feel this is our service. We volunteer to do this service. We do not make any money doing this service.

We ask for a donation that covers the expense of your son on the trip.

This year the cost is:

• If deposit received before May 1st....... $1200.
• If deposit received before June 1st..... $1250.
• If deposit received before July 1st...... $1300.
• AFTER July 1st......................................$1400.

SO.... APPLY EARLY !!!

If you want your son to participate but aren’t sure if you can come up with the full amount, contact us anyway. We can work together to make it happen!
